I have experience in Home Health Quality Assurance, where I reviewed and audited skilled nursing notes, OASIS assessments, physician orders, and plans of care to ensure documentation accuracy and compliance with Medicare, Medicaid, CMS, and state regulations.

I am experienced in using electronic health record (EHR) systems such as Axxess, Kinnser/WellSky. I consistently ensure that patient documentation reflects accuracy, completeness, and continuity of care.

In addition, I have experience in insurance verification and prior authorization, which strengthened my understanding of healthcare workflows, regulatory compliance, and HIPAA confidentiality standards.

I am known for my strong attention to detail, ability to analyze records accurately, and commitment to maintaining high-quality healthcare documentation. I work effectively both independently and collaboratively, and I value clear communication and teamwork.

• Review and audit patient charts for accuracy and completeness
• Ensure compliance with OASIS, CMS, Medicare, Medicaid, and state regulations
• Evaluate Plans of Care (POC), physician orders, and visit notes
• Monitor timeliness of documentation submission
• Identify and correct documentation deficiencies
• Provide feedback to clinicians to improve compliance and charting accuracy
• Prepare quality reports and performance metrics
• Assist with staff education and QA training
