I used to work as an intake coordinator where in my responsibilities includes: 1. Reviewing referral documents from hospital, insurance, and clinic. 2. Checking patient's insurance eligibility to determine if patient is eligible- thru call or simply by accessing insurance portal 3. Data entry- will proceed to input all the pertinent details in our system. 4. Calling and explaining patient if he/she wants to receive home health services. Also, I used to work as a clinical coordinator where I get to work closely with the patients and the following are my responsibilities: 1. Calling hospital, and patient to verify if patient is already discharged from the hospital/SNF and if patient is ready to receive home health services. 2. Creating authorization request for DME, Wound care supplies, and disciplines (SN,PT, OT, ST, MSW and HHA) 3. Faxing and making follow-up for authorization (patient's insurance), MD order (patient's PCP), Supplies (pharmacy) 4. Updating patient regarding the status of his/her authorization, MD order, and supplies. 5. Plotting visits in patient's chart as recommended. Lastly, I also used to work as a Clinical Quality Specialist. My responsibilities are as follows: 1. Reading/reviewing hospital/clinic and authorization documents. 2. Writing and Approving OASIS start of care (SOC), Recertification of care (RECERT), Resumption of care (ROC), and Plan of care (POC)- from OASIS D to OASIS E 3. Doing HHA notes, HHA Supervisory visit, and HHA Care Plan 4. Creating MD order, and 10 day summary (have it sign by license RN) and creating communication notes - as necessary 5. Communicating with the assigned RN and LVN for any discrepancy found in their notes.
