Job Title: HCBS Process Coordinator
Location: Remote
Salary: $6 to $8/hour
Job Type: Full-time, Remote 40/week
Schedule: 9am to 5pm EST

Job Description:

We are seeking a dynamic and compassionate individual to join our team as an Adult Behavioral Health
HCBS Process Coordinator. In this role, you will play a crucial part in facilitating the delivery of Home and
Community-based services for adults with behavioral health needs in compliance with New York state
regulations and guidelines. This position offers remote work opportunities within the state of New York.

Responsibilities:

• Coordinate and oversee the administration of Home and community-based services for adults
with behavioral health needs.
• Conduct comprehensive assessments to determine eligibility for HCBS programs and develop
individualized service plans.
• Collaborate with clients, their families, caregivers, and service providers to ensure coordinated
and effective care delivery.
• Monitor the implementation of service plans, including the provision of behavioral health
treatment, support services, and community integration activities.
• Provide ongoing support and guidance to clients and their support networks, addressing any
barriers to service access or participation.
• Maintain accurate and up-to-date documentation of client information, service plans, and
progress notes in compliance with regulatory requirements.
• Serve as a liaison between clients, service providers, and relevant agencies to facilitate
communication and problem-solving.
• Stay informed about changes in regulations, best practices, and emerging trends in the field of
behavioral health and HCBS.

Qualifications:

• Bachelor's or Master's degree in Social Work, Psychology, Counseling, or a related field.
• Previous experience working in behavioral health services, case management, or care
coordination preferred.
• Knowledge of New York state regulations and guidelines governing Home and Community Based
Services for adults with behavioral health needs.
• Strong assessment, planning, and communication skills.
• Ability to work collaboratively with diverse populations and multidisciplinary teams.
• Excellent organizational skills and attention to detail.
• Proficiency in using electronic health records (EHR) and other relevant software applications.
• Flexibility to adapt to changing priorities and work independently in a remote environment.
