Job Description:
Client coordination + Care
SMS, Video,
Virtual personal support, prompt daily tasks, hygiene, wellness, nutrition, etc
Emotional support to clients
o Follows process to coordinate and manage client care
o Coordinates after-hour phone calls when the office is closed
o Find staffing (caregivers) for open visits which include staffing when a caregiver calls in sick
o Communicate all changes in scheduling to both caregivers and clients
o Updates client and caregiver documentation according to scheduling process via scheduling system
• Quality Assurance
• Maintain client information
o Ensure software is updated and exact at all times
o Maintain tracking sheets to track changes
• Perform other functions as deemed appropriate by the Director or Case Manager
• Maintain confidentiality of all information pertaining to employees and clients
• Manages tasks to be completed in a timely manner
• Assist Director or Case Manager in tasks to maintain quality client care
