Forms / software created

I need forms to be copied / created which need to be integrated so that data entered in one form is also completed in others. The forms require toggles / radio buttons to allow tick boxes to be completed. The software created will need to be able to be provided to other consultants in my industry for them to use but locked so they cannot copy & distribute it.
