Create OASIS SOC, ROC, recertification, transfer and discharge based on the initial assessment and referral with H&P received. (Includes: GG, M items correction based on patient needs for home health care and narrating in homebound status why patient seek home health care) Review and correct 485 generated once OASIS is submitted. Calls facilities and provider’s office to obtain a face-to-face documentation Calls patient to encourage and help them book an appointment with their MD’s. Conduct regular audits of oasis assessments (start of care, recertification, resumption of care and discharge) for accuracy. Completeness and compliances with federal and state regulations.
