• Contacting hospitals or physicians' offices (if needed) to obtain additional documentation required for pre-admission to home health services.

• Managing inbound and outbound calls.
• Update patient's Medical Records and Primary Care Physician's
(PCP) information to EHR/EMR.
• Obtaining signed Home Health Orders from PCP Office (Home Health
Orders such as - Physicians Orders, Therapist and MSW Evaluation Orders,
Transfer Summary, Discharge Summary, DME Request Orders and CMS485.
• Send Home Health Orders via eFax or
• Upload wound images, Admission Agreement and NOMNOC to patient's
episode (chart).
• Review patient's referral.
• Review and validate the Durable Medical Equipment (DME) Request
sent by Clinician ensuring that it has complete and accurate information before
sending the request to PCP Office.
• Call the patient's primary or secondary insurance if the DME
Request has been denied by DME Supplier.
• Update patient's Clinician regarding the status of the DME
Request.
• On training of processing insurance verification using Waystar
Portal.
4 months as Customer Service Representative/ Intake Specialist for
Bluecross and Blueshield of Minnesota and Blueplus (BCBS).
Tasks/ Responsibilities:
• Welcome and
enroll eligible members during the Annual Enrollment Period (AEP) to Bluecross
and Blueshield (BCBS) of Minnesota Health Plan.
• Assisting
members with their inquiries.
• Promote
different health insurance plan of BCBS and guide member which plan suits their
medical needs.
• Provide
computation of estimated monthly premium, deductible, copayment, and maximum
out of pocket amount.
• Check if
members Healthcare Facility, Physician and Pharmacy is in-network.
