Hiring: Community Project Manager

We are looking for a reliable and detail-oriented Project Manager to lead community-based screening and help connect individuals to vital health and social incentives.

What You’ll Do
* Manage Operations: Lead day-to-day participant recruitment and the screening process.
* Coordinate Referrals: Ensure participants are successfully connected to the right services (closed-loop referrals).
* Track Data: Maintain accurate, high-quality records to ensure we meet strict regulatory and Medicaid requirements.
* Support Teams: Act as the central point of coordination for our navigation and screening teams.
* Administrative Tasks: Handle scheduling, document management, email correspondence, and other project or tasks as needed.

Who You Are
* Detail-Oriented: You are highly organized and never let small details slip through the cracks.
* Consistent: You provide steady, reliable work and follow up proactively without being asked.
* Great Communicator: You can speak with stakeholders and tea ---------- mbers clearly and professionally.
* Experienced: A background working with disability service providers or in community-based navigation/social services is a huge plus!
* Independent: You are a proactive freelancer who is comfortable managing your own workload and hitting deadlines.
