Care Coordinator / Case Manager - Healthcare

This position is an important part of the Omni Therapy team, combining research and consultative skills to manage and deliver on time sensitive ---------- re Coordinators work side-by-side with members of the staffing, Q.A. and human resource teams to provide support by managing activity on referrals after the initial visit has already been conducted by a therapist.

Job Requirements:
• 2+ years of healthcare experience as a nurse, therapist, or a role similar to care coordinator / case manager.
• Microsoft Excel, Word and Outlook Experience.
• Must be highly organized and able to multi-task.
• Must be available to work full-time hours: Mon – Fri, 9am – 5:30pm Pacific Standard Time.
