Website Designer from a Template

I need someone to turn my template into a website. If results are good on the first website, then more work will be given. Please do not list the programming languages and other things you know about web design, instead include your past work and for at least 3 of them describe what you did on the website to make it a finished product.

You must include your hourly or project rate to be considered.
