Qualifications
Required:
• Registered Nurse
• Proficient with computers and applications: Microsoft Office Suites, Google Applications, etc.
• Comfortable with Medical Records and Medical Terminology
• Knowledge of medical documentation
• Strong Interpersonal skills
• Detailed oriented
• High school graduate or equivalent
Desired:
• Previous home health/OASIS experience
• Experience in medical field, particularly Home Health
• Experience in medical quality assurance
• Experience with medical electronic documentation
• College degree

Your Day to Day Activities
The Assistant RN Case Manager will assist case managers to plan, organize and direct home health care services.
Specific activities include:
• Review Home Health charts to ensure documentation is complete and accurate
• Perform chart audits on an ongoing basis to verify correctness, completeness, and conformance to standards
• Review assessments of patient to determine home care needs
• Regularly re-evaluates patient nursing needs
• Review the plan of care and makes necessary revisions as patient status and needs change
• Review the care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions
• Identify discharge planning needs as part of the care plan development
• Work with clinical staff to correct issues
• Document common issues to prevent future clinical errors
• Prepare clinical notes and progress notes and updates the primary physician when necessary and at least every 60 days
• Communicate with the physician regarding the patient’s needs and reports any changes in the patient’s condition; obtains/receives physician’s orders as required
• Communicate with community health related persons to coordinate the care plan
