Create Dynamic Form builder, Convert pdf to Laravel blade and vice-versa

The idea is to capture data from clients in dynamic forms. The form which can be sent to any client as a link. They can fill in the information. Once saved, it will be saved in our CRM database. This information can be used to fill the form automatically in Laravel blade and can be downloaded/printed as pdf.
Process:
1. We send an online form to clients,
2. Clients fill the online forms
3. All the information is saved in CRM database when clients submit the form
4. Information is used to fill the PDF form automatically.
5. PDF form can be downloaded/printed.

PART-A
1. Build and design dynamic forms builder/creator system in Laravel with all the functions such as radio button, drop-down, text box, date selector. Country selector etc.
2. Questions can be dependent on previous questions.
For example: Q. Have you travelled to any country?
A: Yes, No
If yes, then the next question might be
Provide further details or name all countries.
3. In some question there may have to add more than one answer. Client/user can add more from the front end.
For example: in below pic- if answer is selected yes, there option to add family member. So the user/client can have option to add more than 1 family members.

In below pic, country visited. So one can visit more than one country. So the user/client should have option to add.

2. It should allow to create as many new forms as needed
3. Any forms can be sent as link a or pdf (if someone wants to print) to client.
4. Once the client fills the form it should save in the CRM database.
5. Create a dynamic form (pdf/word form provided needs to be created in dynamic forms)
6. Integrate if required into CRM (crm is created in Laravel)
7. Include security features such as once the form is submitted people cannot access.
8. While forms is completing, it can be saved temporarily so that one can return later to complete the remaining form.
9. People also be able to sing the form digitally and this signature can be used for Part-B

Note: A form may have 5 or 6 Section (as per below picture) Each section will have heading. The forms should be in a way that client can go to any section and insert/edit data. Show each heading in the left-hand side, once its completed it will move to the next section. People can go back to any section and edit. If not filled its should not able to submit and show error where they need to go back and fill.




PART-B

1. Convert 5 pdf forms to Laravel blade.
2. Use client’s data which was saved in the CRM database, can be deployed to insert relevant information in the Laravel blade form.
3. When printed, it will print as pdf once the data is inserted as per the original pdf design. PDF forms that need to be converted into Laravel blade (all pdf are in the OneDrive link) Form 956, form 80, form 40SP, form 54, form 1221 Word documents that need to be created in to dynamic forms using the system built as per PART-1 (documents are in the OneDrive link)

You will not have access to our CRM or cPanel. Once you create you will give the code and we might implement it ourself or we might ask you to implement though any AnyDesk or TeamViewer. We might give you a CRM design or table that can be used to match with our design. Note: No early release of milestone Work must be completed on time
