Google Forms/Docs to capture data and create reports

I have a fire door inspection checklist based on 13 inspection points that I use to inspect fire doors. It is a simple Yes, No, N/A spreadsheet with an end result of Pass/Fail. This checklist is used for each and every door. At the end I have to consolidate all doors and create a final report with an overview including how many doors passed (percent pass/fail) and a line graph of the overall deficiencies based on each of the 13 inspection points. I'd like to consolidate all of this data entry into an google forms/docs or something that can help me do all of this on the jobsite with a tablet. Attached you will find the final product.
