Looking for a developer to create a web based app and a future mobile app

1) a web app/ crm
2) clients would go to this website, fill information so i can gather their information, submit their photo, sign the waiver, and create an account with password
3) on the admin side, I could see a list of the clients and sort by the categories, search option as well.
