Software Engineer: Custom Web Form For Lead Generation

Looking for a developer who can create a custom multi step web form that meets our needs.

I posted this as hourly to open discussion, but would Ideally structure it as a fixed price when we proceed and have clear scope. I'm looking for THE BEST, not the cheapest.

Here's what we need:
- Multi-step form
- Add expandable text notice on final step
- run scripts on page
- Push to multiple webhooks.
- Fast on page ---------- load time between form steps.
