Developer needed/ Web developer

Hi, we are looking for a developer who can built a quick easy shipping calculator for our website. This is a very small part of our work, we have a database too, we do need someone for the other parts that we are thinking about.

This in essence is a simple test. This video shows you what needs to build ----------

What we would like to know is
1: if you can build it?
2: How much you charge per hour?
3: how many hours will it take you?
4: When can start and have it finished?

What we expect is a working final product, if it has a nice look and feel is a plus ---------- needs to work easily on a cellphone browser.
