Questionaire and Database App developer

I am looking for an enthusiastic and efficient app developer to create a platform to collect feedback from patients. The scores are all well documented but I need them to be in an app form so that users can access them easily. I would like a professional designer and developer who can start with this relatively small project and build it further in the future.

I have many other projects that I would like to develop so further work is readily available.
