Web developer wordpress PHP

Developer needed to build a Customer management page for a custom Wordpress ECommerce site. Only the customer management page is needed for now. This will plugin to an existing larger scale eCommerce system.

This custom page will do the following:
- Page will be a custom PHP wordpress template page
- Page must be responsive to conform to Mobile as well as standard desktop
- Display and capture the customers Mailing and Billing Address information in an Input Form
- Validate the input fields to make sure all the required fields are captured and in the correct format
- Save and Retrieve the information to/from MySQL using Ajax and PHP to MySQL
- Update information from the form to the MySQL DB
- Utilize Jquery Validator or similar to ensure all the required form fields are validated

This is part of a larger project, that if upon successful and satisfactory completion will have more work to ---------- ndidate must be proficient in the following:
