We require software to be written in PHP/JavaScript that will allow a user to enter information and create a report when completed.

The software is numerically heavy and requires many formulas.

How it works is quite simple you can get an extremely good idea from the PDF created. Your version will look identical to this. Once the customer has completed the software it will need to produce a report in PDF or Word format that is simple to understand, use graphs where possible.
