Need a pick up and delivery application

We have laundry shop, we iron.
The client has to choose zip code because for each zip code there will be certain days which are available. Let’s say for zip code 5555 and 666 and 777 only mondains we will do delivery. So when he puts the zip code he will see which days are available.
The client has to select time and hour for us to be able to pick the items up, he will put the time which is ok for us and him.
He will have to tell how many baskets he will give which will let us know if we have enough places in the car or we have to take a bigger car and moreover we will be able to estimate the cost of items.
We need also that the client selects the delivery date and time he wishes. The delivery time will be automatically calculated 3 days after the pick up time by our system that the developper creates. However this delivery option can be optional if it is a complicated calculation for the system.
The system has to be in two languages, in english and french that I can translate.
We also have to handle afterwards if ever there is problem with the system that the developper has to resolve.

I need an order form like:

But we can put an option as the client can be member if he wants and/or "remember me" not to oblige them to fill the same things always. On the second page of order we only have baskets and shirts to be chosen, how many with a +/- buttons menu like theirs. We don't have any prices or online payment as here in Belgium people pay with vouchers so we won't ask them a payment, it is more simplier. For the third page In your opinion which is easier laundrapp or simplewash? As Laundrapp has time and adress on the same page while sipmlewash seperated them. I like laudrapp better in one page but simplewash is easier with big and colorfull calender. Or we can put the colorfull one very in evidence on third page, here in Belgium peopole are not very intelligent , I am a fraid they don't see

The calender has to calculate the the delivery time to 3 days at least from the pick up date

On the other hand I will decide which days are availble for which zip codes. Like if he puts 1000 zip code only mondays will be availble etc...

For simplewash you can put 1000 or 1060 or 1200 as zip code
