Job Description:
We are seeking a detail-oriented and organized professional with a background in Home Health Physical Therapy to support clinical documentation and administrative operations. This role focuses on ensuring accurate patient intake, maintaining complete and compliant records, and supporting efficient coordination between therapists and scheduling systems in a home health setting.
Key Responsibilities:
- Manage patient intake processes specific to home health cases, ensuring accurate and complete information collection
- Maintain and update patient records in accordance with home health documentation standards
- Review clinical notes for completeness, accuracy, and appropriate medical justification
- Upload, organize, and track therapy documentation within the designated system or EMR
- Assist with scheduling of home health visits and coordination of therapist calendars
- Monitor documentation compliance and follow up on missing or incomplete notes
- Support communication between therapists, patients, and administrative teams to ensure timely workflow
- Ensure all records align with home health regulatory and agency requirements
Qualifications:
- Background in Home Health Physical Therapy or experience in a home health clinical setting preferred
- Familiarity with home health documentation, intake processes, and compliance requirements
- Strong understanding of medical terminology and clinical documentation standards
- Experience with EMR systems and scheduling tools
- High attention to detail and strong organizational skills
- Ability to manage multiple tasks in a fast-paced healthcare environment
- Strong communication and coordination skills
