Virtual Assistant Care Coordinator (Non-Clinical CCM)

Position Title: Virtual Assistant Care Coordinator – Chronic Care Management (CCM)
Department: Care Coordination / Population Health
Reports To: CCM Program Manager or Practice Administrator
Employment Type: Full-Time / Part-Time (Remote)

Position Summary

The Virtual Assistant Care Coordinator provides non-clinical support services for Medicare Chronic Care Management (CCM) patients. This role assists with patient engagement, appointment coordination, care plan updates, documentation support, and communication between patients and healthcare providers. The Care Coordinator does not provide medical advice, perform clinical assessments, or make treatment decisions.

The primary goal is to improve patient compliance, increase CCM participation, reduce hospitalizations, and ensure timely completion of monthly CCM requirements.

Essential Duties and Responsibilities

Patient Outreach & Engagement
• Contact eligible patients to explain CCM program benefits.
• Obtain and document CCM consent according to practice protocols.
• Conduct monthly patient outreach calls.
• Assist patients with scheduling appointments and follow-up visits.
• Provide reminders for medication refills, appointments, and preventive screenings.
• Escalate patient concerns to licensed clinical staff.

Care Coordination
• Coordinate communication between patients, providers, specialists, pharmacies, home health agencies, and community resources.
• Assist patients in navigating healthcare services.
• Monitor patient compliance with scheduled appointments.
• Follow up on referrals and specialist appointments.
• Track hospital admissions, emergency room visits, and discharges for follow-up.

Documentation & EMR Management
• Document all patient interactions in the Electronic Medical Record (EMR).
• Update demographic and contact information.
• Assist in maintaining and updating patient care plans under provider supervision.
• Track CCM time and activities for billing compliance.
• Create and manage tasks within the EMR workflow.
