Registered Social Worker

Responsibilities:

• Conduct comprehensive assessments to identify the needs and challenges of individuals and families in the community.
• Develop and implement personalized care plans in collaboration with clients, helping them set achievable goals and overseeing their progress.
• Provide counseling, emotional support, and crisis intervention to individuals and families in need
• Connect clients to available community resources, such as healthcare services, housing options, employment assistance, and financial support programs
• Advocate for clients' rights and coordinate with other professionals, agencies, and organizations to ensure they receive appropriate services and support
• Facilitate support groups and educational workshops to enhance clients' social and life skills
• Conduct home visits and follow-up appointments to monitor clients' well-being and address emerging needs.

Requirements:

• Bachelor's Degree in Social Work, Registered Social Worker (RSW)
• Experience in social work or a related field, preferably in a community setting
• Strong understanding of local social service systems and resources, and knowledge of relevant legislation and regulations
• Excellent communication skills, both verbal and written, with the ability to work effectively with diverse populations
• Empathy, compassion, and strong interpersonal skills to build rapport and establish trusting relationships with clients
• Strong organizational and time-management skills, with the ability to prioritize tasks and meet deadlines
• Proficient in using computer applications and relevant software for documentation and reporting
• Preferably a resident of SJDMC, Bulacan, Caloocan or nearby areas.
