JOB OVERVIEW

Full Stack Engineer — Medical Imaging Platform
Fully Remote | 30hr to Full-Time

About the Role

We are looking for a high-ownership Engineer to own the end-to-end pipeline for moving medical imaging (DICOM) data from hospitals into our platform. It is end-to-end product ownership of one critical capability: DICOM ingestion, processing, PHI cleanup, secure upload, and viewing. You will work directly with senior technical leadership to design and ship the systems clinicians, hospitals, and our internal teams rely on to move imaging data safely and reliably.

What You'll Build
• Node.js backend integration for DICOM ingestion, processing, and storage
• React front end for displaying DICOM
• An Electron desktop application for on-site DICOM processing and upload from hospital environments
• PHI detection and de-identification workflows for DICO ---------- tadata and pixel data
• Secure transfer pipelines from hospital systems into our platform
• Observability, retry, and recovery systems for high-reliability medical workflows

Core Responsibilities
• Own the DICOM feature end-to-end: backend, desktop, web, and integration
• Architect and implement the Node.js services that power ingestion and processing
• Build and maintain the React front end for the DICOM workflow
• Build and ship the Electron app deployed inside clinical environments
• Design and implement PHI cleanup with rigorous safety and auditability
• Communicate clearly through documentation, specs, and async updates
• Operate independently while staying aligned with leadership

Required Experience
• Strong professional experience building production systems with Node.js
• Strong front-end experience with React
• Experience building and shipping Electron desktop applications
• Experience with PHI handling and de-identification in a healthcare context
• Experience integrating with third-party APIs and external systems
• Strong written and verbal communication skills in English
• Proven ability to operate independently with high ownership

Nice to Have
• Mobile development experience, particularly with Flutter
• Hands-on experience with the DICOM standard, including parsing, metadata, and transfer
• Experience integrating directly with hospital PACS or VNA systems
• Experience with medical imaging viewers (Cornerstone, OHIF, or similar)
• Experience with HIPAA-compliant infrastructure and security practices
• Experience processing large media files and high-volume image workflows
• Background in healthcare or clinical software
• Strong documentation and observability habits

How You Work
You:
• Take ownership without waiting to be told
• Treat patient data with the seriousness it deserves
• Ask clarifying questions early
• Communicate risks and tradeoffs clearly
• Enjoy building reliable systems that work in real clinical environments
• Can follow direction while contributing thoughtful improvements
• Thrive in distributed teams across time zones

Engagement Details
• Fully remote role
• Regular collaboration with a distributed engineering team
• Medium-term contract with strong potential to convert to full-time based on performance
