Position Overview

Responsibilities

Responsible for reviewing OASIS-E, chart auditing, and ensuring complete, correct documentation that accurately reflects the Plan of Care.
OASIS-E Review (SOC/ROC/Recert/DC).
ICD-10 Coding.
PCR and ADR preparation and submission.
Clinical Record Review (CRR).
Resource for resolving issues related to patient documentation.
Knowledge of various plan reimbursements (Medicare, Medicare Advantage, Medicaid, Private PPO/HMO Plans).
Review and evaluate all disciplines for quality, cost-efficient care plans.
Lead weekly care plan meetings and assist with quality assurance and improvement efforts.
Chart audit review and addressing areas of concern.
Knowledge of IDPH, CHAPS, CMS Surveys/Audits is a plus.
Familiar with Financial Reimbursement Systems, PDGM, Case mix.

Qualification

Active RN license in the state of Illinois.
Proven expertise in QA for Home Health and OASIS-E review.
Proven experience with Kinnser EMR Software (Wellsky).
Strong organizational skills with a solid understanding of home health care.
Knowledgeable about Medicare home health requirements.
Positive, professional demeanor, excellent team-building, communication, and interpersonal skills.
Highly and consistently organized.
Coding and ICD-10 knowledge is advantageous.
Excellent communication and time-management skills.
