JOB OVERVIEW

We are a U.S.-based outpatient nephrology practice ready to build a full in-house billing
department and we need the right person to make it happen. This is not a maintenance role.
We are looking for a Revenue Cycle Build Lead who has done this before: designed billing team
structures, hired and onboarded billing staff, established workflows, selected tools, and stood up a fully functioning revenue cycle operation from scratch. Once the team is built, this person will
transition into an ongoing leadership role overseeing the department they created.

What you will build:
• Design the full billing department structure for an outpatient nephrology practice — roles, reporting
lines, and team size
• Define job descriptions and hiring criteria for all billing team positions (coder, claims specialist,
payment poster, AR/denial specialist, patient billing VA)
• Lead recruitment, screening, and onboarding of all billing team hires
• Establish standardized workflows for charge entry, claims submission, payment posting, denial
management, and AR follow-up
• Select and configure billing software, clearinghouse, and EHR integrations appropriate for a
nephrology practice
• Build SOPs, training materials, and performance benchmarks for every billing function
• Set up KPI tracking and reporting systems to monitor revenue cycle health from day one
• Ensure all processes and team operations are fully HIPAA-compliant

Ongoing responsibilities (post-build)
• Manage and lead the billing team once fully operational
• Oversee end-to-end revenue cycle performance — clean claim rate, denial rate, days in A/R, and
collections
• Report directly to the practice owner with regular performance updates and strategic
recommendations
• Continuously optimize billing operations and respond to payer policy changes
• Serve as the escalation point for complex denials, coding questions, and compliance issues

Qualifications
• 5+ years of experience in medical billing and revenue cycle management, with a proven track record
of building or significantly restructuring a billing department
• Prior experience in nephrology, dialysis, or outpatient specialty billing strongly preferred
• Deep knowledge of ICD-10-CM, CPT, and HCPCS coding; Medicare ESRD billing rules; and
commercial payer requirements
• Experience recruiting, hiring, and managing remote billing teams
• Familiarity with EHR systems, practice management platforms, and billing clearinghouses
• Strong project management skills — able to plan, execute, and deliver a full department build on a
defined timeline
• CPC, CCS, CPMA, or equivalent certification preferred
• Excellent communication and leadership skills; comfortable reporting directly to a practice owner
• Thorough understanding of HIPAA regulations and compliance requirements

This position supports a US-based medical practice. US healthcare business experience is required. Candidates without US experience will not be considered.
