Keyline Home Care supports and pays family caregivers through Medicaid waiver programs such as Structured Family Caregiving and Personal Support Services. We are growing and seeking a highly accountable Referral Coordinator to drive
case progression from referral to phone screening and through nurse assessment.
What You’ll Do
• Log and track all referrals in
• Contact families and Case Managers promptly
• Prepare families for phone screenings and nurse assessments
• Ensure cases advance from phone screenings and nurse assessments
• Gather required documents to ensure timely case progression
• Ensure accurate provider designation and program placement
• Maintain consistent follow-ups with families and case management agencies
• Advance cases from referral to State Nurse review within 45 days
• Escalate stalled cases proactively
• Maintain clean, audit-ready boards at all times
• You are responsible for keeping cases moving
Performance Expectations
• 25+ cases advanced each month
• No prospect idle longer than 5 business days
• Clear weekly reporting on stalled cases
• Strong conversion from phone screening to nurse assessment to State Review
What We’re Looking For
• Experience in healthcare, home care, or case coordination preferred but not necessary
• Strong English communication skills with the ability to build rapport and connect warmly with U.S. families.
• Highly organized and detail-oriented
• Persistent and professional with follow-ups
