Medical Scribe

Medical Scribe (Remote):
We are seeking a Remote Medical Scribe to support our provider by accurately documenting patient encounters in real time. This position is ideal for someone with a strong medical background who is highly attentive, detail-oriented, and comfortable working in a fast-paced clinical environment.

Minimum Qualifications:
Medical background required (e.g., Medical Assistant, Nursing, Pre-Med, or equivalent clinical or medical office / remote experience)

Knowledgeable in:
• SOAP documentation
• Medical terminology
• Basic familiarity with ICD-10 and CPT codes
• Highly attentive with keen attention to detail and accuracy
• Strong written English communication skills
• Comfortable using EMR/EHR systems and secure communication tools
• Bilingual is a plus
- Candidates without bilingual skills must have a strong medical background or solid clinical experience
• Prior medical scribing experience is a plus.

Key Responsibilities:
Document patient encounters in real time under provider direction, including:
- History of Present Illness (HPI)
- Review of Systems (ROS)
- Physical Examination
- Assessment & Plan
• Accurately record procedures, imaging, and test results as instructed
• Update patient symptom lists and past medical history in the EMR
• Prepare encounters for provider review and final signature (Provider completes and signs all clinical notes)
• Communicate workflow updates with the provider or designated ? Point of Contact (POC) via secure messaging platforms (e.g., WhatsApp for operational coordination only)

Compliance & Confidentiality:
• Maintain strict HIPAA compliance and patient confidentiality
• Follow all clinic policies and documentation standards

• Fully remote position
