Position Summary
The Nurse Case Manager is responsible for coordinating, monitoring, and managing patient care plans across the continuum of care. This role serves as a key liaison between patients, families, physicians, therapists, and other healthcare providers to ensure safe, effective, and timely delivery of services in compliance with Medicare and regulatory standards.

Qualifications
-Registered Nurse degree/license in the Philippines
-Experience with Home health communication
-Familiarity with U.S. healthcare workflows
-Comfortability and professional etiquette with calls to patients, calls with home health agencies/case management meetings, doctors offices(if needed)
-Referral coordination
-Excellent clinical judgment, organizational, and time-management skills
-Ability to work independently while collaborating within an interdisciplinary team
-Strong communication and patient education skills
