About the Role
The Remote Patient Monitoring (RPM) Nurse supports patients enrolled in an obesity?medicine and metabolic?health program through continuous virtual care. This clinician monitors biometric and metabolic data, conducts virtual assessments, reinforces treatment plans, and collaborates with the care team to help patients achieve sustainable improvements in weight, metabolic markers, and overall health.
Responsibilities
???? Remote Monitoring & Clinical Assessment
Review daily biometric and metabolic?health data (weight trends, blood pressure, heart rate, activity levels, sleep metrics, glucose data when applicable).
Identify abnormal values or concerning patterns and escalate according to clinical protocols.
Conduct virtual check?ins to assess symptoms, medication tolerance, lifestyle adherence, and metabolic?health progress.
Document all patient interactions and assessments in the EHR accurately and promptly.
???? Patient Education, Coaching & Support
Provide evidence?based education on obesity as a chronic disease and the pillars of metabolic health (nutrition, physical activity, sleep, stress, and behavior change).
Reinforce medication instructions for anti?obesity and metabolic?health therapies within nursing scope.
Coach patients on using remote monitoring devices and digital tools.
Support adherence, troubleshoot barriers, and encourage long?term engagement.
???? Care Coordination
Collaborate with obesity?medicine providers, metabolic?health specialists, dietitians, behavioral health clinicians, and care navigators.
Communicate changes in patient status, medication side effects, or psychosocial concerns.
Participate in interdisciplinary case reviews and contribute to individualized care plans.
Assist with onboarding new patients and ensuring proper device setup.
???? Compliance, Quality & Safety
Follow all RPM protocols, clinical guidelines, and regulatory requirements.
Maintain HIPAA compliance and protect patient privacy in all communications.
Contribute to workflow optimization, quality?improvement initiatives, and patient?experience enhancements.
Qualifications
Required
RN license in good standing.
1–2 years of clinical nursing experience.
Strong communication skills and comfort with virtual patient interaction.
Proficiency with digital health platforms, EHR systems, and remote monitoring devices.
Preferred
Experience in obesity medicine, metabolic health, endocrinology, primary care, cardiometabolic care, or telehealth.
Familiarity with anti?obesity medications and metabolic?health treatment plans.
Motivational interviewing or health?coaching experience.
Bilingual abilities.
Work Environment
Fully remote with scheduled virtual patient interactions.
Requires reliable internet and a quiet, professional workspace.
May include occasional evening or weekend coverage depending on program needs.
