Adobe/ Brochure/ Web Form Expert/ Designer.

Existing Brochure additions, New Application Form Design, PDF creation and web form Linked to Website. I want to add a cover page and a newly designed application form to an existing brochure. i have the content for the application form I just need someone that can provide and re design layouts ,boxes etc and enter the textual content for me. I need a new adobe PDF that can be emailed and the new brochure linked to the website.
