The Intake Coordinator plays a crucial role in facilitating the initial contact and assessment process for patients seeking services within the organization. They are responsible for gathering essential information, assessing needs, and coordinating appropriate services or referrals. This role requires excellent communication skills, empathy, and attention to detail to ensure smooth transitions for individuals entering the organization's care.
Responsibilities:
• Patient Intake:
Serve as the first point of contact for individuals seeking services.
Conduct initial screenings to gather demographic and insurance information.
Assess patient needs through referrals
Maintain confidentiality and professionalism while obtaining sensitive information.
Welcoming Calls and Communication:
Initiate welcoming calls to new patients, providing them with essential information about upcoming appointments, procedures, and what to expect during their visit.
Address patient inquiries, concerns, and provide guidance on pre-visit instructions or preparations.
Establish a positive and reassuring rapport with patients, promoting a welcoming and patient-centered experience.
• Assessment and Referral:
Evaluate patient needs and determine appropriate services.
Collaborate with clinical staff to assess urgency and prioritize referrals.
Provide information about available programs, services, and eligibility criteria.
Make appropriate referrals to internal programs or external agencies based on assessed needs.
• Coordination and Follow-Up:
Schedule appointments for assessments, consultations, or follow-up services.
Coordinate communication between patients, clinical staff, and management team.
Ensure all necessary documentation is completed accurately and in a timely manner.
Follow up with clients or patients to ensure satisfaction and address any concerns.
• Administrative Duties:
Maintain accurate and up-to-date records in electronic databases
Prepare reports on intake statistics, referral trends, and service utilization.
Assist with administrative tasks such as answering phones, filing, and data entry.
• Collaboration and Training:
Collaborate with other intake coordinators and tea
Participate in training sessions to stay updated on relevant policies, procedures, and resources.
Provide guidance and support to new staff members or volunteers involved in intake processes.
Qualifications:
• Previous experience in intake coordination, case management, or customer service roles is desirable.
• Strong interpersonal skills and the ability to communicate effectively with diverse populations.
• Proficiency in using computer software and databases for data entry and record-keeping.
• Ability to work independently, prioritize tasks, and manage time effectively in a fast-paced environment.
**Remote Work Requirements:**
This position is fully remote, requiring a reliable internet connection and a quiet, dedicated workspace to ensure efficient communication and execution of responsibilities.
Position to be worked Monday - Friday (8am - 5pm Eastern US Time Zone)
