Home Health Quality Assurance Nurse
Job Summary:
A Quality Assurance Nurse evaluates nursing practices within the clinical department to help maximize efficiency and optimize patient care. Their duties include reviewing all clinical documentation, suggesting, and implementing changes, training nurses on new procedures, and ensuring CMS and Agency requirements are consistently followed. The QA Nurse must have at least three years of experience in regulatory readiness, adverse event review, and quality improvement. The QA Nurse should have a track record of using a collaborative team-oriented approach to lead others through change.
Organizational Relationship:
Reports directly to the Administrator and Clinical Manager.
Qualifications:
• Graduated from an accredited school of professional nursing, BSN preferred.
• Has at least 1-year clinical experience in a Home Care setting and Quality Assurance.
• Strong communication and interpersonal skills.
• Axxess home health care experience required
Responsibilities:
• Reviewing medical records to identify any errors or problems with treatment plans or protocols.
• Returning documentation that needs correction by clinicians
• Ensuring adherence to CMS, CHAP, and State standards across all clinical departments and sites by remaining up to date on relevant standards
• Conducting staff training in conjunction with clinical leadership, developing protocols and procedures as needed, and performing intermittent chart reviews and audits
• Training Clinicians as needed on quality assurance and regulatory-related issues.
• Working with Clinical Manager to ensure regular clinical competency verification of nursing staff
• Preparing reports on patient outcomes that are used to improve patient care and nursing practices.
• Working with clinical staff to develop, implement, monitor, and update quality improvement initiatives.
• Developing and implementing a system for monitoring the quality of care delivered by nurses and other staff members.
• Identifying any changes in their condition or behavior that may indicate a need for intervention.
• Reporting any hazards or concerns to the Clinical Manager or other appropriate staff member.
• Displays Flexibility to adjust work based on new challenges or feedback.
Functional Abilities:
• Must be proficient in English and speaks in a clear understandable manner.
• Must be a Nurse
• Communicates effectively.
