About the Role
We are a primary care medical clinic in Texas, USA, seeking a Filipino Registered Nurse or Clinical Virtual Assistant to join our team as a Patient Care Coordinator.
This is a long-term, full-time position for someone who is compassionate, meticulous, and committed to providing exceptional patient care. You will be responsible for coordinating post-visit follow-up, ensuring complete and accurate charting in eClinicalWorks (ECW), and supporting the physician in delivering high-quality, comprehensive care.
Key Responsibilities
Review and update patient charts in eClinicalWorks (ECW) after each visit.
Scribe physician notes and ensure documentation is accurate, complete, and properly coded.
Make follow-up calls to patients to review lab results, check medication adherence, and address ongoing concerns.
Track preventive care items (immunizations, screenings, and chronic disease management) and ensure all are completed.
Monitor and review lab and diagnostic results, coordinating with the provider for timely action.
Communicate with patients and staff in a professional, empathetic, and organized manner.
Maintain strict HIPAA compliance and handle all patient information confidentially.
Qualifications
Registered Nurse (RN) or equivalent clinical experience required.
Minimum 2 years of experience in a medical or clinical environment.
Strong English communication skills (written and spoken).
Proficiency with eClinicalWorks (ECW) or similar EMR systems.
Excellent attention to detail, follow-through, and documentation accuracy.
Must be available to work U.S. Central Time Zone (Texas hours).
Reliable high-speed internet and a professional home workspace.
