Home Health Care Coordinator

Omni Therapy, Inc. is looking for knowledgeable Case Manager / Care Coordinators to handle home health charts on our team. Candidates should have a proven successful track record of working in the healthcare field in a similar role such as case manager or care coordinator.

Care Coordinators work side-by-side with members of the staffing, Q.A. and human resource teams to provide support by managing activity on referrals after the initial visit has already been conducted by a therapist.

Job Requirements:
• 1+ year of recent experience as a healthcare care coordinator / healthcare case manager
• Candidates with 2+ years working with PT's, OT's and ST's in a home health company are welcome to apply as well.
• Experience working night shift is a big plus.
• Must be available to work full-time hours: Mon – Fri, 9am – 5:30pm Pacific Standard Time.
