At HomeLogic Healthcare, we specialize in providing in-home nursing care for medically fragile children. We are seeking an experienced and proactive Home Health Coordinator to oversee daily operations, manage client intakes, and coordinate staff schedules to ensure high-quality care for our families.

Position Summary
The Home Health Coordinator is responsible for the smooth coordination of client services, team management, and staff assignments. This role involves gathering medical records, ensuring compliance, assisting with recruitment, and leading a team of Virtual Assistants (VAs) who support intake, documentation, and scheduling tasks. This is an experience-level position that requires prior home health coordination background, strong leadership abilities, and proven success in managing teams.

Key Responsibilities
Manage client intake process, including gathering and verifying required medical documentation.


Communicate clearly and professionally with families, caregivers, nurses, and agency leadership.


Coordinate caregiver and nurse schedules to ensure full shift coverage.


Track and maintain compliance documents, certifications, and supervisory visit records.


Collaborate with RNs and agency leadership to ensure clients receive appropriate care.


Lead, supervise, and provide direction to a team of Virtual Assistants (VAs) supporting administrative and coordination tasks.


Assist with recruitment efforts by screening candidates, scheduling interviews, and onboarding new hires.


Monitor and resolve staffing or scheduling conflicts quickly and effectively.


Support administrative tasks such as faxing, emailing, and maintaining accurate digital files.



Qualifications
2–3 years of prior experience as a Home Health Coordinator (or similar role within home health or healthcare staffing) is required.


Strong leadership skills with experience managing and mentoring a team.


Excellent communication skills (both written and verbal) are essential.


Strong organizational and multitasking abilities with the ability to work under pressure.


Proficient with scheduling platforms, cloud-based tools, and digital documentation systems.


Ability to work remotely while collaborating with a team in real time.


Familiarity with home health, pediatric care, or caregiver staffing strongly preferred.
