The Case Manager is responsible for ensuring coordination of care for our home health patients. This includes coordinating visits, following up with clinicians, clinical problem solving, and ensuring quality of care.

Qualifications
Required:
• Registered Nurse
• Proficient with computers and applications: Microsoft Office Suites, Google Applications, etc.
• Comfortable with Medical Records and Medical Terminology
• Knowledge of medical documentation
• Strong Interpersonal skills interview new clinicians
• Ability to virtually train new clinicians
• Detailed oriented
• Demonstrates a "growth mindset," with a proactive attitude towards learning and improving skills, coupled with a strong sense of personal responsibility and initiative in achieving goals.
• Ability to work PST hours 9am - 6pm
Desired:
• Previous home health/OASIS experience
• Experience in Home Health
• Experience in medical quality assurance
• Experience with medical electronic documentation
Your Day to Day Activities
The RN Case Manager will assist case managers to plan, organize and direct home health care services.
Specific activities include:
• Perform chart audits on an ongoing basis to verify correctness, completeness, and conformance to standards
• Regularly re-evaluates patient nursing needs
• Review the care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions
• Interview and train new clinicians
• Work with clinical staff to correct issues
• Document common issues to prevent future clinical errors
• Communicate with the physician regarding the patient’s needs and reports any changes in the patient’s condition; obtains/receives physician’s orders as required
Compensation
• $850 per month plus additional bonus incentives
