Front end, javascript, jQuery developer

Hi,
I am looking for frontend, javascript, jQuery expert to create web application (wheel of life - coaching ) for me.
I can send you pic examples. All you have to do is transform it into html+javascript.

The page will include:
1. dynamic chart, wheel with 10 sections. Each sections in different colour with scale from 1-10 (where 1 is centre of the wheel).
1. form, label of each section + input (where user can put number from 1 to 10)
2. page will dynamically fill up the wheel with values from inputs.

So if user put eg. number 7 in every input box, then script will create a perfect wheel filled up with colour from 1 to 7. Outstanding space from 7 to 10 will be blanc.
If user put eg. number 10 in first section then wheel will be 100% filled in etc.

If you know PHP than would be awesome, so we can save data in Database.
