We are seeking a motivated, personable, and results-driven Appointment Setter & Community Outreach Specialist to join our growing home care agency. This role is ideal for someone with prior experience in the home care industry and a strong background in sales and marketing.
You will be responsible for managing inbound leads from platforms like A Home care referrals , DDD referrals and our, as well as actively building relationships with local referral sources such as home health agencies, hospices, and assisted living facilities.
The goal is to convert inquiries into scheduled care consultations and develop long-term referral partnerships within the healthcare community.
Only applicants with home care/senior care experience and a background in sales or marketing will be considered.

Key Responsibilities:
Inbound Lead Management (60%)
• Respond to incoming calls,
• Qualify leads by asking questions to determine care needs and urgency
• Clearly communicate agency services and benefits
• Schedule in-home or virtual consultations for the Care Manager or agency owner
• Follow up on unconverted leads via calls, texts, and
• Keep accurate notes in the CRM or lead tracking system
• Conduct a virtual meet and greet with family and aide
Outbound Referral Outreach (40%)
• Cold call or visit local healthcare facilities and providers
• Build relationships with case managers, social workers, and referral coordinators
• Schedule meetings between referral partners and agency leadership
• Track outreach efforts in a referral database
• Represent the agency professionally during all interactions

Requirements & Qualifications:
• Minimum 1–2 years of home care or senior care industry experience (required)
• Sales, marketing, or community outreach experience (required)
• Excellent communication and follow-up skills
• Comfortable with cold calling and relationship-building
• Tech-savvy with basic CRM,
• Empathetic and confident phone demeanor
• Reliable transportation for outreach, if required
