JOB OVERVIEW

Key Responsibilities:
1. Patient Onboarding & Documentation:
x PF Chart Creation: Ensure that Practice Fusion (PF) charts are created for newly
onboarded patients.
x Document Uploads: Upload key documents within 5 days, including demographic
information, health histories (H&P), wound orders, and previous wound reports.
x Wound Photo Management: After each discipline's visit, upload wound photos to the PF,
ensuring the ruler in the photo is cropped for accuracy.
2. Patient Chart Monitoring:
x Review Patient Visits: Monitor patient visits and keep track of patient information on the
tracker. This includes reviewing weekly visits and cross-checking against the patient
tracker.
x Accuracy & Compliance: Ensure all documents follow established charting protocols and
are accurate, complete, and compliant.
x File Review: Double-check all uploaded files with a second QA member to verify they are
complete.
x Timely File Submission: Send verified documents to Dr. Qais and the patient's agency
within 48-72 hours of the visit, ensuring that strict timelines are met.
3. Follow-up & Communication:
x Follow-Up on Disciplines: Ensure that RN and LVN notes are updated, especially with
detailed information about the wound treatments provided.
x Communication with QA Team: Regularly update the QA team on the progress of
uploaded files and completed notes.
x Report every Friday of the week on the status of patient assignments.
4. Graft Log Entry:
x Verify Entries: Go through each entry in the graft log to ensure that all data is entered
correctly, including patient names, DOB, wound location, ICD Code, Graft Brand, Serial
# and Size.

x Compare the graft log with patient charts and wound care reports to confirm that all
details match and there are no discrepancies between records.
x Check for Typos or Missing Information: Ensure that no data is missing and that there
are no typos in the graft log that could cause confusion or delay treatment.
x Update QA Team: Communicate any errors or missing information to the QA team for
further review and correction.
5. Meetings:
IDT Meetings
x Attend IDT (Interdisciplinary Team) Meetings: Participate in the IDT meetings every two
weeks, which are essential for coordinating patient care across multiple disciplines. This
will allow you to stay aligned with care plans and updates.
x Provide Input on Wound Care: Contribute relevant updates on patient wound care, graft
log, and other QA-related tasks during the meetings.
PRN QA Meetings
x Attend PRN QA Meetings: Attend as-needed (PRN) Quality Assurance meetings to stay
updated on any QA initiatives or new policies. This could be based on the team's needs
or special issues arising within the department.
6. Work Hours:
x Standard Work Hours: Work hours are from Monday – Friday, 10:00 AM to 6:30 PM
PST. Ensure to be available during these hours for any meetings, documentation
checks, and communication with the team.
7. Timely Responses:
x Ensure to Reply: Ensure to promptly respond to any requests, emails, or communication
from the team, especially in regards to documentation or patient care queries.
Maintaining timely replies will help keep workflows efficient.
