**About Us**

Home Heart Florida is a pediatric home health agency based in South Florida. We specialize in Private Duty Nursing (PDN) and the Aide for Medically Fragile Children (AMFC) program, supporting families caring for children with complex medical needs. We are seeking a Philippines-based **Nurse Case Manager** to join our growing team and help coordinate care, support families, and ensure compliance with Medicaid documentation.

**This is a full-time remote position, working U.S. Eastern hours (Monday–Friday).**

**What You’ll Do**

As our Nurse Case Manager, you’ll play a critical role in supporting our clinical operations. You will:

- Coordinate patient admissions and nursing assessments
- Ensure plan of care (POC), verbal orders, and supervisory visits are compliant with Medicaid/insurance requirements
- Communicate with U.S.-based nurses, patients, families, and case managers to ensure high-quality care
- Oversee electronic charting, documentation accuracy, and timely submissions
- Flag any concerns about patient condition, compliance, or scheduling
- Support intake team with clinical reviews and documentation validation

**Requirements**

- **Bachelor of Science in Nursing (BSN)** or equivalent licensed nursing degree in the Philippines
- Active **Philippine Registered Nurse (RN)** license
- Minimum **2 years of clinical experience**, preferably in pediatrics, home health, or case management
- Excellent English communication skills, both written and verbal
- Comfort working with U.S. time zones and healthcare systems
- Prior experience using EMR/EHR or healthcare documentation systems
- Bonus: Familiarity with U.S. Medicaid or home health documentation standards
