NURSE with excellent writing and critical thinking skills

I need a nurse to assist me in the home healthcare industry. This is a full time position as a virtual assistant but you must be very proficient in writing and speaking in English. The position is Monday through Friday - FLEXIBLE HOURS

MUST BE AVAILABLE IMMEDIATELY

One of the important tasks that will be needed is writing comprehensive notes/assessment based off of information that I provide you....

The goal of this assessment/note is to provide a detailed view of an individual's condition and it includes factors like their diagnoses, physical limitations, cognitive abilities, anything that affects their activities of daily living, their social/mental well being, their support system etc. This note is coupled with other documents to determine if the individual is eligible to receive assistance with things like home delivered meals, caregiver or paid family member for assistance, day center for socialization, emergency response system etc.

In short: this note is supposed to prove a case to what is going on with the individual and why they NEED to have these services. (Side note: these individuals are adults with disabilities or elderly and they are living in the home setting either alone, with a family, spouse or renting room etc - they do not live in healthcare facilities).

So, what I need is to provide you some details of the individual (called "member" in my notes) that would include details like I have listed below and you will take it and write the note summarizing and making the case. It does not need to be very long just one page is sufficient.

Name, Age, Diagnoses
Cognitive status, memory, sensory impairments
Living situation
Physical/Functional disabilities and possible tied to a diagnosis/condition (EXAMPLE: Member experiences severe shortness of breath while trying to perform hygiene tasks due to her COPD)
Any hospitalizations/falls etc
Pain/any other symptoms their experiences
Diet modifications/trouble/swallowing/chewing/Nutritional risk
How their conditions limit their independence


Other tasks as needed and time permits but this will be the primary task for now.
