The primary responsibility will involve auditing patient charts and reviewing clinical notes from field staff, as well as other clinical records like OASIS in our EMR system (Data Soft Logic). This role includes completing certification and recertification orders, discharge summaries, and generating comprehensive monthly and quarterly reports. Additionally, it requires coordination with home visiting staff to consult with attending physicians regarding any changes to the treatment plan and processing necessary order modifications. Ensuring that all documentation is compliant and completed promptly is crucial. Effective communication with field staff, physician offices, and colleagues regarding orders, lab work, patient concerns, and other issues is essential. Adherence to agency policies and procedures must be maintained at all times.
QUALIFICATION:
A minimum of (1) one year of experience in a home health setting is highly preferred.
Excellent communication skills.
Required to be productive under strict, small window timeframes with positive results/outcomes.
Knowledge in HIPAA compliance and experience working with Electronic Health Records (EHR ) and Electronic Medical Record (EMR )
Reliable and able to obtain technical requirements.
EDUCATIONAL REQUIREMENTS:
Bachelor’s degree from an accredited school/college of nursing.
TECHNICAL REQUIREMENTS:
Must own a desktop/laptop with I4 processor or higher
Wired internet connection of 5 MBPS or more
Working webcam, headset & mic
