Responsibilities:
?Pay members a friendly health and post check-up follow-up call on a regular schedule.
?Monitor and record unrecorded medication, unrecorded activity for patient record update.
?Promote healthy lifestyles to help lower personal health risks, preventing illness.
?Facilitate effective health care decision-making.
?Improve the quality of care.
?Awareness of the member’s available health and wellness programs, and makes referrals to these programs as appropriate.
?Improve member satisfaction with their health care plan.
?Facilitate appropriate use of health care resources.
?Provides up-to-date health care information to help facilitate members’ understanding of their health problems and possible options.
?Provides assistance by clarifying available options for the member's particular health problem or issue.
?Helps members actively and knowledgeably participate with their provider in their own health care decision-making

Qualifications:
?Active PHRN licensure.
?3+ years of Telehealth coach experience.
?3+ years of clinical experience required, specifically in: Disease Management, Emergency Room, Critical Care, Medical/Surgical Care, Oncology.
?1 year proficiency in Microsoft Office Suite (including Outlook and Microsoft Word).
?Must be willing to work night and start work between 11:00 PM and can choose to work with an 8 hour
?Must have a call-work quiet space at home with Optic Fiber main and secondary internet connection.
?Managed Care experience preferred.
?Previous coaching experience preferred.
?Model candidates have telephone triage nursing experience.
