is attached.
I am writing in support of John Doe’s claim for disability benefits for post traumatic stress
disorder. John Doe has been my patient since 2007, following his return from active duty in Iraq.
I diagnosed John with PTSD in March 2003.
I have personally reviewed John Doe’s medical history. I have also reviewed the in-service
