Clinical Case Manager

FeldCare is a home health therapy provider in California. We are seeking a virtual, full time case manager to manage our patients care.
Responsibilities include:
Coordinate medical services.
Collaborate with healthcare providers to ensure clients receive necessary medical treatments, appointments, and follow-up care.
Document and maintain EMR records: Accurately document all client interactions, treatment plans, progress notes, and outcomes in compliance with legal and ethical Upgrade to see actual infomunicate with assigned clinicians and referring entities.
Respond to phone, email and text communications timely and professionally
Ensure visit notes and charts are maintained to standard.

Prior medical experience is required. Hours are Monday through Friday 9 am - 6pm Pacific Standard Time
