Ebook Formatting

I have an ebook that is almost ready to go. I need a designer to help with the text formatting, inserting graphics, adding front / back covers, and then producing final formats. This job is a cross between copy formatting and design work.

Major steps include:
- Formatting all copy, headings, chapter titles, etc.
- Adding graphics where needed (graphics are all ready complete)
- Adding front and back covers (covers are done)
- Building and adding a Table of Contents from chapter heading
- Misc formatting work with the text

NOTE: Please send examples of previous work. This is specialized work but it is simple.
