This company is a back office of Bridge Home and Hospice located in California. What I do here is to relay the concern of our caller to the Triage Nurse/On-call Nurse and also communicating with the Physicians and Doctors such as scheduling visits and Doctors' appointment. I was a Quality Assurance Analyst and my job was to also check the Home Health charts of the agents. I also do charts outside Bridge Home Health and Hospice (e.g Kamelya, Physician's Choice, Horizon, Denson, HomeAssist, UniCare, and One Health Home Care).
